Returning to daily work and life activities can help an employee’s recovery and reduce the chance of long-term disability. Upon receipt of a completed Fitness for Work Form (included) from the employee’s attending medical practitioner, the employee and employer can develop a Return to Work Plan. This plan outline the employee’s duties and hours able to be performed each day. Purchase of this Return to Work Plan includes a Fitness for Work Form, an Employee Authorization to Release Information Form and 30 Minutes of consulting time with a Salopek & Associates HR Consultant.
